Second, traditional high-THC cannabis possession is decriminalised for personal use under DPR 309/1990: administrative sanctions (licence suspension, passport seizure, mandatory health service report) apply for small amounts, but no criminal record. Trafficking and supply are criminal with up to 20 years for large-scale operations.

Milan's CBD cannabis sector is one of Italy's most developed. Hundreds of shops in the city centre sell CBD flower legally, with quality comparable to European cannabis markets. For THC cannabis, the decriminalisation framework means personal possession is treated as an administrative rather than criminal matter. The practical enforcement is liberal. Cannabis Social Clubs exist in Milan but operate in greater legal uncertainty than Spanish clubs, as Italian legislation on association-based cultivation is less explicit.

Navigli (the canal district) is Milan's most vibrant nightlife and alternative culture area, with multiple CBD shops and an active cannabis social scene. Brera is an upscale artistic district with premium CBD retail. Porta Venezia is the LGBTQ+ and cosmopolitan district, highly tolerant. Isola is young and creative. CBD shops are safe, legal and high-quality in Milan. For THC cannabis, consume privately and discreetly. Do not carry large amounts in public. The Duomo and tourist monument areas have tourist police presence and are not appropriate for cannabis consumption. Milan is a fashion and business city where discretion is culturally appropriate regardless of legality. Legal CBD products purchased in Italy can be transported within Italy but not across EU borders.
